PLUS – enjoy great savings on vision care, prescriptions, chiropractic, hearing, vitamins, fitness, and pet care. Access these benefits nationwide – in person or online – at many convenient locations right in your own neighborhood. The 1st Choice Dental & Health Card is not insurance and not intended to replace your existing dental or medical insurance.

What is the 1st Choice Provider Network?

We have assembled a variety of health and wellness services and vendors who are all committed to providing their patients and customers with excellent care and products. They are conveniently located and often are the very same businesses and practices that you may already use. How much does it cost?

The annual membership for your entire family is just $199. There is no enrollment or activation fee. The family includes a spouse and all dependent children through age 21. You can also choose to make 12 monthly payments of $19.95 which will be automatically charged to your credit or debit card.

How do I join?

How do I get started using my membership?

It’s easy! Once you have registered and paid, our customer care department will send you a complete fulfillment kit including your personal ID card, membership agreement and provider list. Simply select providers to suit your needs and either call for an appointment or stop in. Be sure to take your ID card and you can start saving right away.

How much money will I save?

Your individual savings will vary depending on your card use. Naturally the more you take advantage of our Network providers, the more you will save. Most people find that they save many times that annual cost of their membership. Discounts and services vary , so be sure to be an educated consumer. The dental discounts are off the published usual and customary fees that are charged by most dentists in a particular community. Be sure to discuss procedures options and fees with any health care or service provider before starting treatment. Please read your membership agreement carefully and feel free to contact our customer care department with any questions about any provider.

What if I already have dental insurance?

Remember, the 1st Choice Dental & Health Card is not insurance but a valuable way to get discounts on a variety of health and wellness services and products. Dental providers will accept either your first choice card or your insurance – whichever you prefer. You should never consider replacing any of your current insurance with any discount card. Please discuss questions with your particular provider or insurance agent to avoid any misunderstandings.

Is my membership guaranteed?

Our goal is your complete satisfaction with our service providers as well as having you save substantial money. So, if for any reason you are not happy with your card, we offer a full refund of your membership fee any time within the first 30 days of activation. We cannot refund any payments to providers, as they are directly responsible for any refunds that might be due.
