Using Your Card

• Locate participating providers in your area right on the website or call the toll-free number on the back of your membership card. You may call Monday to Friday 7:00 am to 7:00 pm central time or Saturdays 8:00 am to 5:00 pm.
• When visiting providers or making appointments please identify yourself as a 1st Choice Dental and Health Card member.
• Simply present your membership card to get your discount. Discounted fee schedules that apply to providers in your particular area, are available at our website.
